There are a variety of dental and vision plans available for members of the Arizona Small Business Association and here is a brief description of each of the plans. A "hard copy" can be requested from our office. 

Delta Dental
This is available for groups of of two to nine and composite ten or more employees only.

EDS (Employers Dental Services) (Principal Financial Group)
This is a pre-paid dental plan, meaning that the amounts payable for each procedure are pre-determined. Depending on which list of doctors chosen will dictate the amount of your cost per procedure. Other benefits of this plan are: no deductibles, no claim forms, pre-existing conditions are covered except for procedures in progress. When accessing contracted specialists there is a 25% discount. Orthodontics is included.

Secure Care Dental
For groups enrolling five or more eligible employees you may offer any combination of three available plans as a dual or triple choice. The available plans are a Copay Plan, a PPO Plan and an Indemnity. The plans can either be voluntary or contributory. The plan also give prior credit for satisfaction of waiting periods met under your employer's previous qualifying group dental plan.

Note: The annual maximum benefit per year for the copay and the indemnity plans is $1500.00
Note: The Copay and PPO have out-of-network provisions so one can see any dentist.

American Dental
This plan is also based on a reduced or discount fee concept. Unlike indemnity or a pre-paid dental plan, you pay a reduced or discounted fee only for the dental services you actually receive. For example, a contracted dentist provides two cleanings per membership year and the second is no charge. Your routine examinations and x-rays are also no charge. Rates are as follows:
